Demand and Need Lead Community Concepts to Develop Unique Case Manager Position

In partnership with Community Concepts’ Head Start program, the Family Services Department of Community Concepts has developed a Case Manager program to serve Head Start families in Oxford and Franklin County.

Christine Hufnagel, Director of Family Services at Community Concepts says the new position was developed as a result of agency wide discussions, “The staff of our Head Start programs were seeing a high number of children in their classrooms with qualifying diagnosis or challenges but little to no community support to help these families manage the challenges.”

The agency views the work of this team vital in developing a strong engagement with children ages 0-5 years old.  These Case Managers will be responsible for intake/assessment, identifying the nature of problems, developing a plan of care and coordinating the delivery of services.  The ideal applicant will have experience with this age group.

“The Head Start population is unique in that is it geared toward preschool aged children,” said Hufnagel.  “Having managers that are experts in this age group will make the services more effective and targeted.”
