Children’s Case Management

We are currently accepting referrals in Androscoggin, Oxford and Franklin counties.

Case Management staff work with families to identify and address any medical, educational, social or other needs. Case managers will assist families in developing and maintaining family teams, creating crisis plans, creating individualized service plans, advocating, and accessing community services and other resources. Families have a choice of case management agencies.

Funded by: Department of Health and Human Services, MaineCare

Community Concepts’ Children’s Case Management Program provides several services:

  • Intake/Assessment
  • Individual Service Plan (ISP)
  • Development
  • Coordination/Advocacy to access services and resources identified in the ISP
  • Monitoring of the ISP
  • Evaluation of the ISP

Children and their families will receive services at a level appropriate to their needs in a coordinated fashion with their other providers.

Case Managers will work with families to determine eligibility.
